ABC Healthcare Company is a nonprofit multispecialty physician practice offering primary and specialty care to approximately 600,000 patients via a network of community-based clinics and preferred hospital relationships. It is a leader in value-based models of care; about half of ABC’s patients are under a risk payment contracts with commercial payers as well the Medicare Pioneer ACO.
ABC Health has a robust Enterprise data infrastructure including a fully integrated data warehouse, which receives claims data from insurance payers, Electronic Medical Record Data, as well as third party risk prediction models including APR DRGs.
The Director of Population Health, who oversees the operations and implementation of the ACO program, has asked you to create a Dashboard (updated monthly) in order to track ABC Healthcare Company’s performance with its ACO initiatives.
Your Dashboard should take into consideration:
Audience: Who are you communicating this data to? Will the Dashboard help the Director visualize performance? What is the data story that you need to convey in order to assist with strategic decision making and operations?
Strategy or goals of the ACO: How should an ACO measure its performance? Which KPIs are relevant? Are the metrics you choose to display aligned with strategic objectives? Will you display Goals or Targets?
Metrics: Consider Process and Outcome Measures; Are any of the Metrics lagged? Can you display Trend or Performance Variance Year over Year? Are there any caveats to the data?
Your Dashboard should include:
-24 months of data points, especially for any year-over-year comparisons you choose to display
-At least 3 different types of data visualizations (i.e. charts, tables, graphs etc.)
-KPIs (both Qualitative and Quantitative)
-Year to date measures as well as a monthly performance snapshot.
-At least one process and one outcome metric
-Goals and Targets
-Performance Variance from the previous year
-Encoding or Thresholds
-The dashboard needs to fit on one legal size page. Submit the dashboard as an Excel Spreadsheet.
I suggest that you re-read the Eckerson Article before you get started, re-familiarize yourselves with the Triple Aim and Value Based Contracts, as well as do some research on what ACO are and what they care about in terms of performance goals.
This is your chance to be creative and consider what kind of story you want to tell about the ACO’s performance. You will need to create dummy data to supply the visualization elements. I’m not concerned about the “validity” of the actual numbers you use, but think about seasonality.
In addition to the Dashboard, write a short description (no longer than 500 words) of your dashboard development process, your reasoning for selecting your data elements, any challenges you encountered, and how you think this dashboard will be an effective tool for the ACO. Cite any references you used for your write up.
